RESTORE2™: Recognising and escalating deterioration in residents
Buckinghamshire’s NHS organisations and its Council encourage your home to participate in a free training offer to help support your staff and manage your resident’s health.
RESTORE2 (Recognise Early Soft-signs, Take Observations, Respond, Escalate) has proven very beneficial in helping all health and care professionals spot deterioration and to take positive steps to support their health for both non-COVID-19 and COVID-19 related illness
The RESTORE2 tool has been designed to help health and care professionals to:
– Recognise when a resident may be deteriorating or at risk of physical deterioration
– Act appropriately according to the resident’s care plan to protect and manage the resident
– Obtain a complete set of physical observations to inform escalation and conversations with health professionals
– Speak with the most appropriate health professional in a timely way to get the right support
– Provide a concise escalation history to health professionals to support their professional decision making
The British Geriatrics Society (BGS) recently released a new guide, COVID-19: Managing the COVID-19 pandemic in care homes offering key recommendations to help all care home staff and the NHS staff who work with you to support residents through the pandemic.
One of these recommendations advises staff in care homes in the practice of taking vital signs to use the RESTORE2™ tool to recognise deterioration in residents, measure vital signs and communicate concerns to healthcare professionals.
Together the Oxford Academic Health Science Network, Buckinghamshire Council, Buckinghamshire NHS Trust and Buckinghamshire Clinical Commissioning Group are wanting to work with and support care home staff in detecting and appropriately escalating deteriorating residents by providing facilitated online training in RESTORE2™. RESTORE2TM mini is a condensed version of the full RESTORE2TM tool – ideal for introducing care and residential homes that are currently unable to measure vital signs to the concepts of soft signs and SBARD structured communication.

RESTORE2 Mini
• Aimed at residential care homes to enhance the carers confidence with recognising
early signs of deterioration in a residents usual behaviour, known as ‘Soft Signs’
• This training will include scenarios which should help to increase a carers
competence in using the SBARD tool (Situation, Background, Assessment,
Recommendation, Decision) to enable them to safely and appropriately escalate any
concerns
• Duration: 1 hour
RESTORE2 Full
• Aimed at Nursing homes, although suitable for any staff who are competent with
taking observations such as blood pressure, pulse, respiration rate etc.
• This training will incorporate the NEWS2 tool, which then provides a risk score for
the resident which can support appropriate and timely escalation
• This training will also include scenarios which should help to increase a carers
competence in using the SBARD tool (Situation, Background, Assessment,
Recommendation, Decision) to enable them to safely and appropriately escalate any
concerns
• Duration: 1.5 hours
