Frankie Brown MSc in Health Sciences (Gynaecology) Cert Ed in Higher Education. Recently retired nurse, Frankie has worked in General Practice for many years, mostly in the area of Women’s’ Health, Sexual health, contraception and Respiratory,.  She has for many years had a specific interest in asthma and was a Senior Trainer at the National Respiratory Training Centre now known as Education for Health.

She was also a Senior Lecturer at Oxford Brookes University where she ran the Contraceptive and Womens health modules, she left to concentrate on her Independent work.  5 years on the Oxford City PCT Executive Board gave her a great insight into the wider picture of the health economy both positive and negative.

Social Prescribing

The Buckinghamshire Social Prescribing Steering Group have a membership consisting of Clinical Commissioning Group, County and District Councils and a selection of voluntary sector organisations, working together to deliver and embed social prescribing in Buckinghamshire.

Social prescribing is a way of linking patients and residents in health and social care  with sources of support within the community. It provides referrers with a non-medical referral option that can operate alongside existing treatments to improve health and well-being.

We already have services in Buckinghamshire that offer social prescribing such as Prevention Matters and Live Well Stay Well and there are numerous services run by the voluntary sector that provide non-medical support for people in Buckinghamshire.

The Social Prescribing Steering Group ensure that these services are working together with a common aim and that the right people (patients and health care professionals) know how to access them at the appropriate time.

In many of our GP practices in Buckinghamshire we have a Care Navigators.  These are practice staff who can offer patients non-medical support when it is appropriate.

Care Navigators are skilled to signpost patients to local community services which may support them with their needs.  The Care Navigator role is key to the Social Prescribing strategy as they are instrumental in ensuring that patients find out about the services that can help them.

The Social Prescribing Steering Group are working with Care Navigators to find out about the most common issues that our patients present, so that we can work to make sure the services they need are easy to find.

Long Term Conditions Education and Training for all Health Care Professionals in 2019

Since 2017, we have organised a successful programme of education and training to support your work with care and support planning and diabetes.

Moving into 2019 we are extending this to other long term conditions and developing a programme of education to meet the competencies required to deliver basic management of Care and Support Planning, Diabetes, COPD, Asthma, Cardiovascular and Mental Health.

We can also offer ‘bespoke-in-house’ education in your practice.

The Diabetes Bulletin is changing to the Long Term Conditions bulletin and will continue to be published monthly and is a great way to keep up to date and informed.
